Booking online is a three step process:
Step 1) Click on the date/time of the appointment in the calendar below. Use the < or > keys next to the date to navigate to a day that has appointments available.
Step 2) Enter your personal information and click the arrow on the right (under "Step 2 of 3").
Step 3) Enter your contact information and click the Finish button.
You will receive an email indicating you have requested an appointment. When the appointment is confirmed by our staff, you will receive a second email indicating the appointment has been scheduled.
It is important to check your Spam, Junk, and Promotions (Gmail) folders for the appointment reminder. This email will come from the email address: firstname.lastname@example.org
If you are scheduling as a NEW PATIENT you will receive an additional email from email@example.com. Please check your Spam, Junk, or Promotions (Gmail) folder for this important email.
